Clinical case management experience including aftercare planning, benefits advocacy, community linkage, specialty referrals, and interdisciplinary coordination.
Documentation and systems experience including Epic for Kaiser clinical documentation, Lucet for outcome tracking, and Salesforce for CERI service coordination.
